How to Change my Primary Care Physician?

Call your insurance company.
Change the name of your PCP to Dr. Uzma Jafri.
If your insurance does not have her listed, please check out insurances accepted at Reliance Medical, PLLC and consider changing insurance if having a trained physician take care of you is one of your healthcare goals.
Some insurance requires a 30-day period before approving the new PCP to see you. If you don’t want the 30 days, you may receive a bill from Reliance Medical, PLLC for the full cost of the visit.

How To Sign Up

Provide copies of insurance cards (front and back) to fax to 1-623-328-8978 or give them to Director of Nursing

Fill out forms

Find out from the Director of Nursing when Reliance Medical, PLLC is scheduled to round

Call your Insurance company.

Change the name of your PCP to Dr. Uzma Jafri.
